I had in interview with them today. Like you said, I did the phone interview first and then I filled out paperwork and did the computer questions and then after waiting about 45 minutes, a manager came and got me. I think the manager already made up his mind he wasn’t going to hire me because from what I’ve read, you have a second interview that same day. I didn’t. He said he would get back to me within the week to schedule another interview if I was selected. I expect to get the rejection letter within a few days which is fine because honestly after talking to that guy, I am not sure I would want the job anyway

Same thing happened to me recently in the Omaha store. I walked out of the interview when they started questioning a 7 year old bankruptcy and when they asked me if i thought i was a bad person for doing it.

Unfortunately I have no idea where you might be able to find one. I have the one that came with my mothers machine, but thats a matched set I would be unwilling to part with. E-bay would be my best suggestion.
